How is CFS/ME diagnosed? Why is there a controversy around exercise intervention and CFS?

The criteria for diagnosis of CFS has changed - PEMS is key.

(Please note that when I go off on a tangent in relation to options, specifically surgery, this is regarding musculoskeletal injuries, not CFS. There is no surgery for CFS!).
